Sick Benefits

Active Members may apply to receive financial assistance for medical expenses not reimbursed by another source. Members may apply twice per calendar year for assistance with a maximum benefit amount of $750 per application. The sick benefit rates are reviewed annually and adjusted according to current economic indicators.

Background

CUPHSONAA was originally established to assist School of Nursing graduates in a variety of manners. At the time of inception, Social Security and unions did not exist, modern employee benefits were not offered, and nurses were working six and a half days per week. Wages were low and should a graduate lose their job or become ill, they were placed in a precarious position. CUPHSONAA stepped into the void and created financial resources for its members though monthly stipends and sick benefits. To this day, CUPHSONAA continues to provide monthly stipends and sick benefits to its members in need.

Eligibility

Members who meet the following criteria are eligible to apply:

  • Classified as an Active Member
  • Submitted expenses must not be reimbursable by another provider or source.
  • Submitted expenses must be for the current calendar year

Application

Sick Benefits Application

To apply for the sick benefit, a completed application must be submitted with the following supporting documents:

  • Certification from a treating physician
  • Receipts for all expenses listed on the application, expenses submitted without a receipt will not be considered for reimbursement
  • Receipts must demonstrate the expense was paid in part or in full, documentation for expenses paid in part must list any partial payments the applicant has made toward the expense

The sick benefit is classified as a gift and is not subject to income tax. 

Application decisions are made on a case-by-case basis. 

The information contained therein is strictly confidential and known only to the Finance Committee and the Executive Director.

Eligible Expenses

Eligible expenses include, but are not limited to:

  • Medical, dental, and healthcare related expenses, including:
    • Lab work
    • Radiological services
    • Eyeglasses and contacts
    • Physical therapy
    • Hearing aids
    • Insurance co-payments
    • Prescribed medications
    • Prescribed assistive devices
  • Transportation via ambulance

Ineligible Expenses

Ineligible expenses include, but are not limited to:

  • Transportation costs, excluding ambulance transport
  • Health insurance premiums
  • Home remedies
  • Food
  • Over-the-counter medications
  • Over-the-counter supplies or devices
